This card sended to me by my friend already flashed, but with wrong rom. It is works but only through 10.4.8 to 10.6.3. I used nvcmdflasher - Mac version of nvflash - to flash Quadro FX 4500 rom from Download section. But nvcmdflasher won't flash rom with original 009d Device ID, so i changed in rom to match GPU ID 0095. After that card start work normally and booted to Mac OS X 10.7.1 Lion Server without problem, recognized as Quadro FX 4500 with 512 MB VRAM. No kext hack needed.
